JEFFREY MARK POLACEK age 53, passed away suddenly February 18, 2014. Devoted husband of Raisa (nee Sulym) for 35 years; loving father of Elicia (Lou), Katrina and Nikki; cherished grandfather of Louie and Issa; beloved son of Barbara Kleihnen and John (deceased); dearest brother of Debbie LaSalle (Ralph); dear uncle of Andrew and Adam and friend of many.
